On January 23, 2025, Walnut Creek SNF Opco, LLC d/b/a Walnut Creek Nursing Center (Walnut Creek), Dayton, Ohio entered into a $243,000.90 settlement agreement with OIG. The settlement agreement resolves allegations that Walnut Creek employed an individual who was excluded from participating in any Federal healthcare program. OIG’s investigation revealed that the excluded individual, a licensed practical nurse, provided items or services that were billed to Federal healthcare programs.
